Agree Brian Dolan OBE, RMN, RGN regarding age based cutoffs. Just a surrogate marker until we get better at scoring frailty which I must say we are getting better at. Otherwise we have very little data to understand the demand and create the best services for our frail population

Every little helps in a complex system. No single magic bullet. What we want is change in processes to deliver this hour’s work this hour or at least today’s work today for UEC.

Absolutely! When do we as an NHS, stop focussing on the ‘symptoms’ of flow issues and address the out of hospital resources, processes and 24/7 availability? Short term fixes are NOT sustainable

There are 2 reasons why things don’t happen. ‘I don’t know’ or ‘I don’t want to’. And there may be very good reasons for the latter. Why does the HEE not increase the training places in geriatric medicine? Or is the uptake poor? We need a honest discussion.
