If we're having a public enquiry into the , why aren't we having one into the regular systemic failures leading to victims being killed by perpetrators? These deaths are PREVENTABLE. To fail to act is INEXCUSABLE.

I've seen a number come to the realisation by speaking to professionals who are informed or other victims.
