Leukemic mantle = CD23-, FMC7+

CD5-/CD10-:
Hairy cell leukemia = CD123+, BRAF+
Hairy cell leukemia variant (now called SBLPN) = CD123-,BRAF-

B-cell PLL = CD5+/-, TP53

CD5-/CD10+:
Leukemic follicular cell

82/male/ presents with fever and a rash !

Blood counts are ⤵️ so he undergoes a bone marrow examination!

The blasts look like 👇🏼

On FCM they are + for CD4, CD56, CD123, HLA-DR and negative for CD34, MPO, CD19, CD10.

DIAGNOSE:
A 29-year-old male presented with the skin lesions and he had no systemic symptoms such as night sweats and weight loss. Skin biopsy showed diffused dermal infiltrates with blast-like cells extending to the hypodermis.

#hemepath IHC. Just an image of normal distribution of CD123 in reactive LN. Plasmacytoid dendritic cells. Endothelial cells are also positive.

Cutaneous MZL class switched showing the cute IgG4 positive PC’s at the periphery of one of the aggregates. I also like the CD123 trick.

As many of you guessed, these cells are indeed basophils!
Basophils are seen in the 'blast' gate, have low side scatter, and express dim CD45, CD13, CD33, CD38 and CD123. Unlike plasmacytoid dendritic cells, basophils do not express HLA-DR.
