While we need further study in this area of sarcoidosis, our finding is particularly relevant as a recent study showed that memory formation is mediated through TLR9. Perhaps too little TLR9 activity results in neurocognitive deficits and too much TLR9 leads to fatigue. 6/

Although we found that mitochondrial DNA-TLR9 interactions only partial mediates fatigue in sarcoidosis, our work suggests that chronic innate immune activation represents a potential biologic mechanism for the clinical manifestation of fatigue in this disease. 5/

We found that high levels of mitochondrial DNA, a ligand for the innate immune receptor TLR9, are not only elevated in the plasma of patients with sarcoidosis, but are also associated with various domains of fatigue, including its physical and mental aspects. 4/

Using the GRADS study cohort, which enrolled patients with sarcoidosis across 10 different institutions across the US, we conducted a multi-disciplinary translational study to investigate the biology that drives the development of sarcoidosis-associated fatigue. 3/

Sarcoidosis-related fatigue remains one of, if not the most, common symptom experienced by patients with this disease. Even after treating all known reversible causes of fatigue, patients with sarcoidosis continue to experience high levels of this debilitating symptom. 2/
