One possible reason why PAs became popular with consultants and the hospitals was the issue around continuity. The current rota system is a barrier to continuity. Can we bring back the firm system?

Absolutely and that’s what British medicine was once about; ability to process the information and create a clinical impression. We at BAPIO Training Academy have started Sherlock Holmes series to impart critical thinking.

It’s likely that consultants appointed in the last5+ years were exposed to this model and now perpetuating this poor practice

Sadly they are learning these bad practices from consultants. Not only this is costing huge amount to the nhs but also causing immense harm to pts. Hippocratic and Charak oaths; medical profession seems to have forgotten

Insightful piece. We are witnessing how #excellence is fast disappearing from the NHS, in all areas. Clinical excellence is replaced by mindless investigations which is just a mediocre way to practice medicine.

Didactic teaching has very little place in PG education. Maybe UG needs some but not at the cost of clinical exposure

More pts you see better doctor you will be! Some F1s are Med to believe that they are not supposed to see pts on the wards! Unbelievable

It multi factorial- system issue; our educational leaders maybe need to have better understanding of #learning but consultants need to empower trainees to see pts and make a judgment under supervision. My simple take is- can surgical skills be learned by simply observing in OT?

Sadly this generation doesn’t believe in seeing pts. The system has made them scribes and note taking docs. Time to change and Royal College of Physicians must take lead

If that were agency consultants then it’s the right decision. Let’s start saying no to expensive agency locums; lot of them don’t even have proper training and qualifications. It’s a surprise how the nhs has allowed this
