CABG vs PCI for multivessel CAD? We must be cognizant of when CABG may improve outcomes over PCI. A nuanced topic with several trials (🔑: BARI-2D, FREEDOM, BEST, EXCEL). Calculating a SYNTAX score stratifies patients by complexity multivessel disease. SYNTAX Trial, NEJM 2009 ♥️

狭心症患者で、糖尿病＋多枝病変はCABG推奨です。基本的にはCABGは生命予後を改善する治療、PCIの予後改善効果は残念ながら不確かです。

冠動脈の見た目がPCIに向かないから手術を考えると言うより、CABGを前提にしてカテーテル治療でも予後が変わらなさそうならPCIを選択するのが理想だと思います。
