CTO ostial LAD, 3 DES starting from LM, no reflow starting from the proximal LAD stent
Aspiration done several times but still the same
Injection from the aspiration catheter showed this
Echo didn’t show even 1 mm of pericardial effusion
Patient died on table

New OCTOBER Trial analysis in JACC Intv led by Lene Nyhus Andreasen ;
Unintended stent deformation (USD) was associated with very poor outcome - But if USD was identified by OCT and corrected the prognosis was fine.
USD was identified in 18% of LMCA PCI cases (!)
Causes were equally

1/ Early graft failure. Most would usually say treat the native…but what do you do if the graft is still patent but degenerative and may be causing embolic phenomena?

My opinion on the use of MCS has been the following since #NCSI
1. Routine use in AMI-CS (routine doesn’t mean 100%)
2. Selective use in HF-CS
3. Highly selective use in HR-PCI
4. Limited to no role in AMI w/o shock (I have never placed an MCS to “prevent” CS).

This is why and how we cut the #rota driveshaft and remove the sheath to help remove the stuck burr.
Cut near device. Pull on the sheath and get it out. Now u have the space to get GE on it or go around it with wire/balloon

2 yrs s/p ViV TAVR with 26 E Pro+ in a 25 CE with MI presentation- MG 9 in June 23, now 35. Huge sinuses, circular deployment.

1. Start with a short 5 minute warm-up.
2. Do 4 minutes of sprinting at 85-95% of your max HR
3. Rest for 3 minutes (walking)
4. Repeat this cycle 4x

This takes around 30 minutes, which is less than an aerobic workout & longer than a traditional HIIT workout.
